Diane provided a letter with information pertaining to their taxes such as estimated payments made and charitable contributions. To the letter, Diane attached all of the appropriate tax forms she received (W-2,1099-INT, etc.) and the income and expenses for Jack's business, Cougar Construction. Thank you so very much for agreeing to assist with preparing our 2023 tax returns!With the retirement of our prior accountant, we were worried that we would not be able to find someone to prepare our tax returns on such short notice.In this letter, I will provide some information to help with the preparation. Here is our contact information: Jack and Diane Mellenkamp 1313 Mockingbird Lane Cincinnati, OH 45255(513)3471111 My date of birth is July 1,1981 and Jacks is July 4,1981.My Social Security number is 412345670 and Jacks is 412345671.I work as a Program Manager with Groctor & Pamble and Jack is selfemployed as a Construction Foreman.Jack is covered under the medical insurance I get through G&P and we both had coverage for the whole year.We do not wish to contribute to the Presidential Election Campaign and we do not own or trade any crypto currency. The following tax forms Jack and I received are enclosed with this letter: Form W2 from G&P Form 1099INT from Third Fifth Bank Form 1099DIV from G&P Form 1099B from G&P summarizing a stock sale Form 1098 Mortgage Interest Statement from Rocketz Mortgage Form 1099NEC from IM Homes for Cougar Construction The income and expense information for Jacks SingleMember LLC, Cougar Construction, is attached.Cougar Construction has an EIN 238917567 and its NAICS business code for box B of the Schedule C is 236100. Our prior accountant mentioned that Jacks business is a qualified trade or business eligible for the Qualified Business Income Deduction.The principal service offered by Cougar Construction is Residential Construction. All of the business assets associated with Cougar Construction are fully depreciated.Cougar rents an office space for the business and the business address is: 1776 Independence Drive Cincinnati, OH 45255 We do have some itemized deductions that we are hoping to take with the tax return.We made payments to the State of Ohio in the form of withholding on my G&P wages ($5,531.52) and Hamilton County real estate taxes ($1,700.00).Also, enclosed is a mortgage interest statement Form 1098 which shows the interest we paid to Rocketz Mortgage.The big item we deduct every year are our charitable contributions.Jack and I donated $26,000 to our church this year and have a letter from the church confirming the amount.I did not include the letter as our prior accountant never needed it, but she told us to keep it with our tax returns in case the IRS ever questioned our deductions. We made timely federal estimated payments of $20,000($5,000 each quarter).If we have a refund this year, please apply it towards next years taxes as we always have to pay estimates because of Jacks self employment income.
