You have been working at a service that provides care to individuals across a range of age groups. Some of your service clients are those with disabilities; others are from aged care who live in their homes. Your clients have a diverse range of requirements and have individual care plans where they require a team that may include health and allied health workers, support workers and family. This team will help implement the care plan.
