To address one of the issues you raise regarding the financial incentives of cutting corners, it would seem that some sort of counteracting incentive to reinforce so many days of safe behavior and outcomes might be effective. Such as a VR inspection schedule of the use of specific safety equipment where everyone in compliance would receive a bonus and those not in compliance would miss out on the bonus and be required to go through a retraining. Companies operating under capitalistic systems are by design going to attempt to maximize profits, but safety and the associated costs of accidents and corresponding mitigation (and litigation) should be factored into daily operations somehow. Companies will spend a fortune to clean up a mess, pay court costs and settlements, and for a marketing rebrand, so it seems they would consider spending a fraction of these costs to promote and incentivize safety and compliance.
