You work in a large textile factory as secretary to the Safety Officer

(male or female, as you choose). He is responsible among other things for ensuring that adequate precautions are taken against dust, exposed machinery, fire, and hazards such as falling objects.

on the general standard of safety in the factory. He must briefly investigate
the causes of the recent accidents, and find out how the employees
regard the directives of the Safety Committee. (He is free to consult
other members of the Committee.) If necessary there could be sanctions
against employees who break safety rules, such as suspension (which
might cause a strike). There could also be an education programme to
inform people about the vital need for safety (though this might be
very expensive to arrange).
You have assisted the Safety Officer to carry out his investigations
and have been asked to compose a suitable report, which he will sign,
for submission to the Safety Committee. Inventing minor details to add
realism, write your draft report. (Note: No expert knowledge of
factories or industrial safety is expected.)
