Given that systems of quality control have been established in audit firms and that auditors’ performance is monitored by the RSBs, audit failures should be a thing of the past.

Explain briefly:

(a) Why audit failures still occur, and

(b) The checks which society has in place to ensure that auditors responsible for causing harm to others as a result of substandard work do not go unpunished.
