I know it's a best practice, but must I always prepare a formal risk management plan for every project? So many of the risks I have identified in the past never actually happened.

A. The size of the risk management plan should be appropriate for the size, length and importance of the project.

B. The time devoted to the risk management plan should be 10 percent of the length of the project.

C. The risk management plan should always be done, unless you are too busy.

D. When possible, do a risk management plan to avoid all possible risks.
