Description

FMEA is to eliminate or reduce the chance of process failures. In this project you'll conduct a FMEA of a familiar process.

Review Figure $12\mathrm{.}1$ top$-$down flowchart of the activity of going to work.

Note: The sub$-$processes are provided for information only. Your personal experiences may differ somewhat from the example.

Use the answer sheet provided for this project to complete a FMEA on the process of going to work.

Refer to the process steps that are pre$-$listed in column one. In column two, list what could go wrong $($the potential failures$)$ at each of the process steps. There is space on the form to list two failures

$($one per row$)$ for each process step.

For each failure listed in column two, determine what would happen if the failure occurred $($the effect$)$ and record this in column three. Each failure should have an effect recorded.

Using the risk assessment matrix provided with the answer sheet, rate the likelihood of each failure you've listed in column two. The likelihood rating is a numeric score, ranging from $1-6.$ Put the

likelihood score for each failure in column four.

D$.$ Put the consequence score for each effect in column five.

Using the risk assessment matrix provided with the answer sheet, rate each failure listed in column two as high, medium, or low. This rating is obtained by determining where the frequency and

consequence scores for the failure intersect on the matrix. Record the rating for each failure in the last column.

Select two of the high or medium failures and write a short description of what could be done to prevent each failure from occurring.
