Preparing and Coordinating Plans
Execution of project planning will provide all stakeholders with detailed information that assists the project team in understanding exactly each person's responsibility and the desired outcome of the project. First, define the team's goals, what the group hopes to achieve, and the purpose behind the intended project. The leader, or project manager, over the project will break down the goals of the project into outcomes that are measurable. For this particular project, our group will assess ways to cluster patient care in order to decrease a patient's length of stay within the hospital and also improve patient outcomes. Next, using current scholarly research, team members will begin to evaluate ways in which clustered care can be achieved. It is essential for all members of the team to determine who will be responsible for which parts, having all members informed of all information upfront, and also to be organized. Once this is done, the team members will need to evaluate potential risk associated with the project. Next, the team will need to develop training materials for staff and schedule time for meetings to evaluate the success of the project and make adjustments. Finally, the team members need to work closely with the training manager to schedule employee education through modules and/or in-person training.

Below will outline the scope of work for clustering the care of patients to reduce noise and improve patient satisfaction and outcomes.
Scope of Work:
- Determine the types of care that should and can be clustered
- Determine the standards of care protocols
- Determine and develop a way to measure these outcomes
- Delegate staff who can aid in the committee planning and implementation of this plan
- Plan committee meetings to explain and share the measurable outcomes for goal-setting
- Plan for the go-live date with a hospital protocol within six to nine months from the first committee meeting.

Stakeholder Management Plan
Stakeholder involvement is a crucial part of implementing this project into practice. All stakeholders involved should feel like they are engaged in all forms of planning and carrying out the plan. A major key to success for the project would be communication amongst all parties to ensure they are achieving their individual goals and the team is achieving its goals for the project's implementation. There will be bi-weekly meetings that each stakeholder should attend either in person or via Webex/Zoom to share what they are working on and where they are assisting with the planning and implementation of the project. Once the project has been shared with staff, there will be monthly staff meetings to determine what is and is not working for staff on the success of clustered care needs.

Risk and Issue Management Plan
Identifying project risks is imperative to aid in the success of the project outcome. While each project has risks, the cause of risks varies depending on the complexity or type of project. Identifying the risks will allow the Project Manager to prepare the team for potential weaknesses, threats, and unexpected events that may disrupt the project at any stage. Anticipating these risks will allow the team to be proactive in mitigation versus reactive, potentially causing project delay or failure. Identified risks for clustering patient care within the ICU include ineffective communication, continuity of patient care, and push back from staff. Effective communication is non-negotiable regarding project success; this must range from the CNO to family members. Expectations must be clearly stated to all stakeholders, including the overall project goal and implementation strategies to promote project success. Not only is verbal communication essential within this project, but also the patient's chart. Documentation within the patient's chart must be definitive, reflecting what care was performed, when it was completed, and the outcome; this information will help with qualitative data upon project completion. Additionally, another identified risk is the continuity of patient care. As a Project Manager, one must ensure that all staff involved in direct patient care are aware of expectations and standards going forth with the implementation of the project. For example, Nurse A is excited about this project to increase patient outcomes and goes above and beyond to ensure care is clustered to promote sleep hygiene. At the same time, Nurse B is resistant to change and does not cluster care, therefore not promoting sleep hygiene, ultimately contributing to ICU delirium. Lastly, while not only specific to clustering care within the ICU to improve noise reduction, resistance to change within healthcare workers is a common challenge to overcome. Effective strategies to implement in anticipation of resistance include communicating the vision of the project as well as benefits from implementation, including and empowering staff members throughout the process, providing support to staff during implementation stages, and celebrating the progress as well as improved patient outcomes. Although risks are inevitable, project success is still achievable despite the challenges that may arise.

Reflection and Application of Knowledge
Putting together the project management plan assisted in developing an understanding that not only does it decrease stress and steps for the staff when care is clustered, but it also improves the quality of care for the patient (Maidl-Putz et al., 2014). For this project, patients are not continuously disturbed during their stay and less stress is exhibited, especially during night shift. Due to the implementation of clustered care, causing a decrease in the disruptions to patients, their length of stay may decrease and also their overall health may improve. With this decrease in the length of stay, patients are benefited due to short recovery times and also they are not seeing a hospital bill as high.
Establishing the project management plan required the utilization of the geographical, physical, and the nursing practice environment. With this establishment, it allowed for more communication amongst stakeholders to carry out the plan of clustering care. Establishing the plan in forms of tables and precise bullet points allowed for easy visualization amongst all stakeholders. Going forward in practice, the implementation of these tables and precise bullet points will allow for anyone reading the display to visually picture the plan in a clear and precise way.
