(1) Her birth date is January 29,1989.
(2) Her Social Insurance Number is 654321123.
(3) She is paid on a weekly basis (52 periods in a year)
(4) Her regular rate is $36.00? hour.
(5) In this pay cycle, she worked 40 hours at her regular rate plus 2 hours overtime.
(6) Overtime rate: 1.5 times regular rate (will not be included in calculating vacation pay)
(7) She is paid 6% vacation pay with each pay period based on earnings, not including allowance.
(8) She is paid $50.00 each pay period for a car allowance to compensate her for using her own
vehicle while driving on company business.
(9) She is paying 10% of Net Taxable Earnings as federal income tax
(10) She is paying 20% of Net Taxable Earnings as provincial income tax
(11) She pays $15.00 per pay period for union dues and $100.00 per pay period for RRSP.
(12) She has authorized the employer to deduct $5.00 per pay period for her parking fees.
