This includes discussion of -Any progress made towards these goals -Difficulties encountered when working towards these goals -Confirm and prioritise the most important needs, goals and preferences Did the student plan in consultation with the older person how they will negotiate and access resources or services including: - Resources required to deliver identified services - Community support agencies to achieve the identified goals? 4 Did the student plan how they would provide support and respite to families and or carer's? This includes identifying specific support issues affecting families and or carers and referring appropriately
