You are required to complete a practical activity with a person to enhance their awareness of care support in the care setting. You should identify a person you are or have worked with. Considering the person's interests, plan a suitable activity that will enhance their experience of care support. You should organise your assignment as follows Planning - A. Observe the chosen person and determine appropriate activity suitable to their needs B. Explain the reasons for your choice of activity C Show knowledge of materials / resources that may be needed to carry out activity D. Develop implementation plan E. Show an awareness of health and safety and best practices throughout the planning and carrying out of the activity

Consultation & Communication Consultation with the supervisor and person should be evidenced. Discuss the the importance of working as part of a multidisciplinary team Demonstrate effective communication throughout activity - verbal & non-verbal Show an ability to communicate effectively with team members, supervisors or any relevant personnel

Person Centered Care - Demonstrate best practices that will enhance the person's independence and dignity, Briefly describe how the activity was carried out Evaluate the impact that the activity had on the person Promote good practice and respect for diversity in the care setting
