Mother: Kim, age 26; daughter of Joe. Kim and Bill have a son, Josh, age 3. Father: Bill, age 30; partner of Kim. Bill Is ATSI. Grandfather and carer: Joe, age 52. Joe was born In Italy and English Is his second language Kim, Josh and Joe live together In a public housing home in a Victorian regional town. Joe is the primary caregiver for Kim and Josh. Kim has a severe mental illness that is stabilised with medication, but she is not able to take complete responsibility for Josh's care. She sees a mental health worker at the Community Mental Health service attached to the local hospital twice a month. She has a home support person who comes to the house to help her with the activities of daily living, including to help her try to take some responsibility with Josh and household chores to assist her father. She would like to live with Bill again but can't decide if she wants Josh with them or not. Up until about a month ago, Bill was living with Joe, Kim and Josh but due to his substance misuse (alcohol) and family violence towards Kim, Kim has told Bill to "get out and don't come back until you're sober". There is no Victoria Police history of family violence. Kim appears to minimise the family violence and blames the alcohol. Bill is Aboriginal and proud of his culture. Even though Bill is no longer in the home, he has been making an effort to see Josh as much as possible, either at the home or a park/playground and often talks to Josh about culture and country. Bill has legal matters pending due to an assault that occurred at the local pub, he is being harassed by debt collectors and is unemployed due to absenteeism due to his substance misuse. Bill is couch surfing with people he knows however this will not be a viable option for him much longer. Bill wants to reunite with Kim and Josh. He is unable to read or write English however he can speak English but at times becomes confused. Joe is the primary caregiver of both Kim and Josh and generally manages well. At the time, Kim's illness and Bill's substance misuse were not well managed so their lives were chaotic. Joe took them all in 18 months ago. He has recently suffered a severe and incapacitating heart attack. He is currently still in hospital and attending a heart rehabilitation program and is recovering well; however, he has been under considerable stress and is worried about both Kim and Josh. Josh lived with Kim and Bill for the first 18 months of his life. Josh is an active 3-year-old who is not yet toilet trained. Josh attends a childcare facility 3 days a week and is very attached to the childcare staff. He has been at several childcare facilities in the past but has been at this one for the past 7 months and his behaviours at childcare have improved. The staff have been very supportive during Joe's illness and the childcare facility provides a stable environment for Josh. They have a pickup and return service for toddlers only, at the centre. Josh particularly loves water play at childcare and enjoys being in the play pool in the backyard at home. Childcare staff have observed him startle at very loud noises and can be withdrawn at times. Childcare staff are assisting Josh to use the toilet they have found him very responsive and tries hard to learn new things. Recently a complaint was made to Child Protective services about the smell in the home and behaviours of Josh. The complaint stated, "there is a three-year-old kid living in the house, I don't know how he can survive that stench", it also included concerns that Josh is seen in dirty clothing, does not appear to be washed, has run out of the house a few times and there is lots of yelling coming from the house, sometimes this is directed at Josh. The reporter said that the old man is not there at present but even when he was home these things were still happening. You have investigated this complaint and made the following case notes: An overpowering smell of stale air and urine on entering the house. Josh was observed to be wearing dirty clothing and was running around the house. Kim advised that she finds it extremely difficult to control Josh's behaviour and some days she can't get out of bed due to being depressed You observed food in the fridge that was appropriate, and Kim advised she cooks for Josh and herself every night, but you have also noticed packages of take away food left around.
