A heavy gas oil pump in the #2 crude oil unit at an Exon refinery has a leaking inboard seal. Shift Supervisor Tom has written multiple work orders to have this seal checked. It has been worked on 3 times over the last 4 months (replaced twice). Tom is concerned that there is more wrong with the pump than just the seal. If a seal on one of the hot oil pumps were to completely fail it would cause a significant spill and possibly ignite. He intends to communicate his concerns via an email to the crude units maintenance supervisor (Carl), the safety supervisor (Paul) and with his own supervisor (Ryan - operations superintendent).
Should Toms letter be formally or informally written? Explain your choice.
What information or data should Tom provide? Is there any research to be done or sources of additional information he could investigate?
Should Tom visit in person with any of the people he intends to email first? Explain why or why not.
Taking creative license with this scenario, take on the role of Tom and compose a short email explaining the situation to the three people mentioned.
