A. On February 26, 2003, ABC Cor, the largest employer in a small metropolitan city in the US, assigned maintenance employee Bob O. to repair an overhead crane in the main shop during business hours. He was having trouble reaching the area he was repairing and choose to climb out of the scissor lift to reach the area. Bob was not wearing a harness and fell on to the concrete floor from the scissor lift which was 20 feet in the air. He sustained head injuries in the fall and lost consciousness.
