A sentinel eventis defined by the Joint Commission as an unexpected occurrence involving death, serious physical injury, or psychological injury. Examples include patient suicide, wrong-side surgery, op/post-op complication, and medication errors. Such events are called "sentinel" because they need immediate investigation and response. The Joint Commission identified the following sentinel events through self-report, media, complaint, survey, CMS or state reports over an eleven-year period for hospitals that participated in a study (Table 4).
