A young New Brunswick worker's leg was dragged inside a conveyor at a wood pellet facility, and he later perished from his wounds. The conveyor had malfunctioned twice in the days prior to the disaster, and during repairs, a guard was removed but not replaced, according to WorkSafeNB's inquiry.

The young man did not know that the conveyor's guard had been taken off during the day shift while he was working the night shift cleaning it. The worker was pushed into the moving conveyor because she was unable to see that the guard was missing due to poor lighting and cleanliness in the area.
