He is having difficulty completing his schoolwork since returning to school. He is not disruptive but is unable to finish his assignments in the allotted time (his classmates finish their assignments without difficulty). When he does complete his work, he makes several careless mistakes. Although he is still passing his classes, his grades have dropped, and he seems to daydream a great deal. The teacher reports that he needs instructions repeated frequently in order for him to be able to complete his schoolwork. He enjoys gym and does well in the class. Teachers report that his mind wanders constantly and they have to call his name or wave to get his attention.

The boy's parents have observed these behaviors at home, but have learned to work with them. If they supervise him and the assignments that he is working on, he can complete his homework. His parents have also noticed that his assignments contain many careless mistakes. The parents also said that he struggles to get ready for school in the morning; they have to keep encouraging him. His bedroom is unorganized and cluttered. He also loses things all the time. His parents say he is a happy child who enjoys playing with his siblings and friends. They note that he does not like school, except for his gym class.
