An NFL football team held a fan fest prior to the football season. The fan fest included games, activities, interactive play equipment, food, and more. Prior to the fan fest, a rental company had spent the day dropping off and installing inflatable amusement devices. A 19' inflatable slide called the Titanic was placed outdoors and anchored to the ground. The rental company left the device to be operated by interns working for the organization.

The fan fest started around 3:00 pm. At around 5:30 pm, catastrophe struck. A total of 26 patrons (mostly young children and some pre-teens) made their way to the top of the Titanic inflatable slide. While at the top of the device, it started to sway. The patrons decided to enhance the swaying by moving side-to-side with the device. The device eventually tilted significantly enough to collapse. All 26 patrons fell off the device. 6 of the patrons required hospitalization. 2 of the patrons eventually died from head injuries.

5. People's perception of risk is directly related to their experience with it. Considering most people avoid it, under-appreciate it or have never been exposed to it, few people realize when they are in danger.
