An organizational chart is a visual representation of organizational structure. Please view the attachment for the organizational chart of Chaos Organization. In an organizational chart we can see the hierarchy including chain of command, departmentalization, and in this case, span of management (# of employees that report to an individual). The lines indicate the reporting relationship. This organizational chart is "chaotic" because it doesn't make sense let's describe problems, identify the management principle involved, and suggest how to fix it. Please don't attempt to identify every problem in your post. Pick one area or position (put that as the subject) and explain the problem (why it doesn't make sense), try to identify the management principle (span of management, chain of command, departmentalization) involved, and how you'd fix it. Once we feel comfortable with our changes, maybe someone can make a new organizational chart that reflects the changes.
