Assignment Summary/Objective: Use a current OHS incident/tragedy to discuss the incident causes, apply legislation and provide detailed recommendations on how to prevent it from recurring.

Steps: Discuss/analyze the chosen incident. Your discussion must cover the topics and answer the questions below:

1. Discuss the main hazards/risks associated with the workplace/industry of the chosen incident (i.e., hardware supplier, manufacturing industry, pulp, paper mills, etc.)
2. Identify the hazards/risks associated with the particular task/equipment that the injured worker was performing/using (i.e., Welding, working at heights, pulp machine, etc.)
3. What are the potential adverse health effects (i.e., injuries) that could result from the risks identified in question 2?
4. Discuss and analyze the causes of the chosen incident, according to the Ministry of Labour investigation.
5. Apply all relevant legislation (over and above what is already mentioned in the case):

- What are the legislations that were violated by the employer/worker that have led to the incident?

- What are the legislations that must be complied with, in order to implement corrective actions to prevent such incidents in the future?

- Note: When referring to legislation (Acts and Regulations), you must mention the section/code numbers (e.g., section 25(2)(d) of the Occupational Health and Safety Act and section 25 of Ontario Regulation 851 R.R.O. 1990).

6. Provide detailed recommendations on how to prevent the incident from recurring in the future. Discuss all the corrective and preventative actions that must be implemented in the workplace along with any relevant OHS programs that must be developed (e.g., fall protection program).
