INSTRUCTIONS
Written Work
You must analyze a specific job selected (Well Testing in Oil and Gas) and create a general Job Hazard Analysis (JHA) relevant to that process. The job should include a minimum of 7 individual tasks but no more than 10. The JHA will identify:
Type of job and associated tasks
Actual and potential hazards
The type of harm or property damage associated with the hazards
The initial risk factor using the risk matrix provided which includes:
Likelihood
Severity
Control methods as per the hierarchy of controls
Residual risk factor using:

Severity
Once the JHA is completed, you will use it as the basis to develop a Safe Work Practice (SWP) for that job. At a minimum, the SWP should include the hazards, safety considerations, required personal protective equipment (PPE), tools/devices, and include steps that will guide the worker through the job from start to finish. The SWP should be designed and written in such a way that it is easy to read, understand, and follow.
The written submission will include the following:
A cover page
Table of contents
Introduction
A summary detailing the JHA, SWP, and Orientation process and how they all tie together as part of an effective health and safety program.
Appendices that will contain the JHA, SWP, and Orientation outline
References/citations where applicable
The written work will be submitted as a .pdf file and in APA style format
