• undertake a simulated evaluation of the program (Appendix C)

You are required to submit the:

1. Consultation plan
2. Program plan (power point and handout/template of plan appendix B)
3. Program Evaluation

Part 1

Specific instructions:

Plan for the consultation phase of the program development

Reflect on the consultation with relevant stakeholders prior to you designing this program according to the client group you have selected, or if you have found the program on the Internet, come up with a hypothetical one (Appendix A).

• List the names of the consumer groups and stakeholders relevant to the client group
• Record the strategies you have used to engage and consult with each group (real or hypothetical)

Record a summary of the feedback gained during the consultation (hypothetical or real information)

Part 2

Develop a PowerPoint and handout (appendix B) which could be used to present the program to staff and Board members to explain it if one does not exist already

• Description of the program and type of service involved
• Objectives of the program
• Source of funding of the program

Part 3

Evaluate and review the program

Use the template provided (Appendix C) to undertake a (hypothetical) review and evaluation of the program after completion. You will consider the success of the program according to the following criteria-
