Checklist 1: Reflective journal This checklist outlines the criteria you will use to mark the student's reflective journal. All criteria described in the checklist must be met. The checklist also contains benchmark responses for you to use when assessing to ensure reliability of judgement. Document future plan of behaviour to address improvements Final result Comments S U/S * U s us Part 2: Personal development plan (PDP) You are required to complete a personal development plan aimed at improving your professional practice. You may base your PDP on your learning in the classroom through lesson content, discussion, role-plays, people's past experiences or the simulated environment. The plan must include 3 SMART goals and implementation strategies that are: Specific - clear and unambiguous Measurable - allow you to track progress and know when the goal is accomplished Achievable - realistic and attainable Relevant - directly relate to improving your professional practice Time-bound - activity date/duration/end-point. The three goals must address the following topics: One goal must be related to implementing lessons learned from an experience documented in the Part 1 reflective journal. One goal must be related to investigating and using a current or emerging industry development to extend expertise and improve practice. One goal must be related to self-care and potential support networks, both workplace-based (e.g. peers, supervisor, specialists, managers) and in the community (e.g. community support groups, personal supports, professional/specialist support services or associations). Implementation strategies may involve informal or formal training and development opportunities, specialist advice, networks and professional associations as relevant. Record your answers in the PDP template provided on the following page. An example response has been provided to help guide you. A checklist is also included that will be used by your assessor to mark your PDP. Use the checklist as a guide to satisfactorily complete your PDP template.
