You need to identify all the possible risk and plan for the countermeasures. a) List down all the possible issues/problems associated that can bring all the possibility of causing serious injuries and fatalities in confined space. b) Define the confined space and find the related cases happens in Malaysia and the other countries. c) Can you find any similarities of the causes of the incidents in Malaysia and US/UK/Europe? d) Develop the risk register table for working in confined space in Malaysia.
