Create a data entry screen to add new employees and customers. It will be one screen that will be used by human resources to enter new employees and by the sales department to enter new customers. You will need the following:

Salutation (Mr, Mrs, Ms, Dr, etc.) combo box

First Name text field

Last Name text field

Address text field

City text field

State text field

Zip text field

Phone Number text field

SS Number text field

Type Radio Buttons (employee, customer)

List Of People - JTextArea

Enter 3-4 people into your app and append them to JTextArea on the bottom of the program.
