A university is hosting a scavenger hunt for teams of between 3 to 5 people. All team members must wear matching shirts with the universities logo and a team name. Each team leader must place only one order for all of the teams shirts. You have been asked to design the screen that will be used for taking the shirt orders.

The information that must be collected from the user on the order screen includes:

1. Customer name
2. Customer address, city, state, zip
3. Customer phone number
4. Customer email address
5. Customer payment method (credit card, Paypal, or check)
6. Shirt type (short sleeve, long sleeve, or sleeveless muscle shirt)
7. Shirt color (white, blue, or gold)
8. Shirt size (small, medium, large, or extra large)
9. Number of shirts in each size
10. Team name
11. Delivery option (pick up or ship to customer)
12. Express shipping option

Each shirt costs $10 and the form must display the total order cost. It must provide buttons to place the order, to clear the form, and to cancel the order/exit the application.
