This assessment will be referring to the incident with Craig that we began to investigate in Day 3 Assessment.

Now that you have informed the PCBU of what actions are required to manage the scene and you have planned your investigative processes, it is time to investigate why this happened.A Workplace Health and Safety Incident Notification Form has been completed and will assist you with your investigation.In addition, you have obtained the following information from site:

Workers JSA for task being conducted

Witness statements about incident

Training records of all involved

Use the evidence taken from site and the table below to help organise and collate your information.

a)Complete the following table to collate your investigation data/information

What happened: an overview

Provide an overview of what happened.

Nominate the type of notifiable incident - was it death, serious injury or illness, or 'dangerous incident'?

When did it happen?

Date and time

Where did it happen?

Incident address

Details that describe the specific location of the notifiable incident - for example section of the warehouse or the particular piece of equipment that the incident involved - to assist instructions about site disturbance

Who did it happen to?

Injured person's name, salutation, date of birth, address and contact number

Injured person's occupation

Relationship of the injured person to the entity notifying

How and where are they being treated (if applicable)?

Description of serious injury or illness - i.e. nature of injury

First aid initial treatment of serious injury or illness

Where the patient has been taken for treatment

Who is the person conducting the business or undertaking (there may be more than one)

Legal and trading name.

Business address (if different from incident address), ABN/ACN and contact details including phone number and email.

What should be done to secure the scene?

Action taken to preserve the scene and by who.

Who notified the Regulator?

Notifier's name, salutation, contact phone number and position at workplace.

Name, phone number and position of person to contact for further information (if different from above).
