After emergency shelter or permanent housing resources are identified, the assigned life coach will assist the clients with linkage to health services monthly or as needed to assist participants with improving overall mental and physical health and wellbeing

Outcome 2

After being placed in an emergency shelter, temporary housing, or permanent housing, the clients will meet with the life coach weekly and comply with SSI, SSDI, or employment directives to increase income and gain financial independence to maintain their monthly expenses independently.
