When these unfortunate accidents occur, an investigation is required to examine the causes of the event so that it can be avoided in the future. - Put yourself in the perspective of a Human Resources Generalist working at a manufacturing plant with 100 employees. - Today, at 15:30, a container of nearly red-hot, upper-control-arm forgings was overturned. The hot forgings fell on a worker who was helping the drop forge machine operator. The worker suffered third degree burns over 20 percent of his body. Though he is expected to recover, the worker will lose most of his right arm, right ear, and sight in his right eye. Describe the investigative methods and tools you would use to investigate this incident. - Some pertinent facts: The worker received all mandated health and safety training but did not receive specific training regarding the drop forge as it is not his regular work area. o. There are visual aids and reminders for health and safety awareness of the machines in the area where the accident occurred. o There are 3 other employees who you believe witnessed the incident. o The supervisor witnessed the incident, performed first aid and facilitated the employee reaching the hospital. The scene has been secured, machines shut off in the area, and the area blocked off from all employees. - Based on this information you will describe your approach to an investigation of this accident.
