Doctor Office: This database is for a typical doctors office. An office has a name, an address, and multiple phone numbers. An office also is of a specific type: express, full service, emergency. Each office has many doctors, a doctor works for one office. Doctors have a name, a specialty, and a salary. Valid specialties are pediatrician, cardiologist, and podiatrist. We will keep track of patients. Each patient has a name, address, date of birth and account balance. Any time a patient sees a doctor, a record needs to be generated with the doctor seen, the date, an amount charged for the visit and the primary diagnosis (valid types include: fever, cold, surgery). Patients can visit any doctor in any office. We will also keep track of patient payments. Each payment consists of an amount, date and a type of payment (cash, check, credit card).

Create an ERD from the following requirement. Use Chen notation with cardinality and participation notation and make a separate diagram with UML notation. If you make an assumption that is not in the requirements, please note this on the ER.
