Each student should create a form to be used in the evaluation of the performance of a coach/sponsor. This form can be a checklist or narrative but should reflect the degree to which the coach has met the performance standards listed in the job descriptions developed by the student. The forms should be of a quality suitable to be included in the Coaching Handbook the student is developing. This evaluation form should also include a two-page, single-spaced, and word-processed rationale for the items included in the form, as well as a description of how the form will be used. For example: Will coaches be evaluated annually or at regular intervals? Who is responsible for completing the evaluation? What is the appeals process? And so forth.
