Explain a time in your life (preferably in terms of a workplace, student organization, etc.) that you were part of a group that was responsible for making a decision and it did not go well. In addition to explaining the situation, identify which of the pitfalls to group decision making from your textbook led to the problem AND how that led to the problem.
