Fluffy and Bushy (F&B) is a US based veterinary clinic that treats animals in the US. There are several clinics throughout the country.

They assign each clinic a clinic ID, and they need to store the clinics street address, state, and zip code. Each clinic has one or more employees assigned to each location. An employee can only be assigned to one clinic at any given time. If an employee changes clinic location, the system is update to accommodate the change and no history is kept. F&B needs to keep track of each employees name (first and last only), and all phone numbers the employee can be reached. Each clinic has an assigned employee who is also a manager. That employee can only manage the clinic they are assigned.
