For this assessment you will need to describe and analyse a situation/scenario that you have experienced in relation to working with an underperforming learner. If you do not have a scenario involving a learner, you may want to think about a scenario with a peer or colleague. The main requirement is that you choose a scenario/situation which did not go as well as you would have wanted, as this will give you plenty of scope to critically analyse the situation and identify areas for improvement
