This information should be addressed in a short introductory paragraph that precedes your questionnaire. Step 2: Write and explain Next, it's time to write. Each question in your questionnaire should be carefully crafted to obtain the information you are seeking. For each question, include a short explanation that describes why you are including the question and what the question is attempting to measure. Using Sources Along with relevant Study.com course materials, your questionnaire must include at least two credible, scholarly sources from outside the course. At least one source must be from a scholarly, peer-reviewed journal. The sources must be cited using APA format, and should include a mix of both primary and secondary sources. As a reminder, primary sources are those that report on the original results of the experiment or study, while secondary sources include comments, discussions, or observations about primary sources.
