When a donor makes an eyeglasses donation, their basic personal information (name and address) is collected and stored. Donated eyeglasses are given an ID number and are then checked for the following info: Gender, size, lens info. All information on donated eyeglasses, including the date it was donated, is stored. When someone is of need of eyeglasses, they submit a request form, including their personal info (name and contact) and a prescription from their doctor. Volunteers match eyeglasses requests with the eyeglasses inventory data to see if there is a match. If there is a potential match, the recipient comes in to test the glasses. If a pair of glasses works, s/he is given the glasses and receipt with information on the glasses (initially collected). The receipt date as well as who received the eyeglasses is noted in the donated eyeglasses files. At the end of each month, thank you notes are created and sent out to that months donors. Every six months, a report is generated to give to the organizations director. Included in the report is a summary of the types of glasses received and the number of glasses distributed.
