Create a Performance Appraisal form by blending three different methods of appraisal (See Fig 8-2 on page 201, such as Rating Scales, Critical Incident, Narrative Method, etc...) and use the form you create to conduct a self-appraisal of your overall performance in this class. Must be typed.
- Student must create an original performance appraisal form.
- Do not exceed 2 pages
Step 1- Create the form including the following-
Section 1-Graphic rating Scale or Bars (p.202-03) You must include at least 5 job-related areas to evaluate your performance (don't guess at what this means, read the chapter to understand what I'm asking here).
Section 2- Critical Incident (p. 201) Include this section that keeps a record of your highly favorable and unfavorable actions in this class. Should include at least 4 with detailed descriptions and dates.
Section 3- Narrative (p. 202). Include this section for a detailed narrative describing your overall performance. The narrative should include evaluative statements. Narrative section should be 3-4 paragraphs.
Step 2- Once you've created your new performance appraisal form, don't forget to conduct your self-appraisal of your overall performance in this class. Fill out the form that you created. You will type your appraisal of yourself on the actual document you created in Step 1. Be sure to be objective and honest in your appraisal
