Additional Background Information on Grace: In order to assist you in writing a comprehensive assessment, the following details about Grace have been created: Grace is a 25-year-old woman who called the Community Counseling Center's intake unit requesting services. She provided the following background information: Grace is employed as a residential care supervisor at a group home for adolescents. She worked there for three years following her graduation with a degree in social work from the local university. Grace reports that she faces a great deal of pressure and some life decisions that have brought her to the agency for support. Recently she reports an incident at work where she became angry with her boss and the Director of Human Resources encouraged her to seek counseling. She denied that her job was in jeopardy. Grace lives with her boyfriend of three years, Mason. He also works at the same facility as Grace. She reports no contact with her family of origin but admitted that this is "a big part of what I need to talk about with a counselor." She reported that her parents divorced when she was an infant and she and her mother lived with her maternal grandmother until she died at age 5; her mother also died when Grace was preadolescent, and she has had no contact with her father for many years. She has no siblings and only has sporadic and superficial contact with extended family members. Grace does not identify with a particular ethnic tradition due to her limited family relationships. Grace reported no substance abuse problems but has a history of mental health treatment when she was a child and teen. She reported that she is not on any medications. The client was raised in a non-religious home but attended church with a friend in high school. She found comfort in the protestant church and has continued attendance and involvement periodically over the years. She has not attended services in the past 12 months. Assignment Information: You are to use the handout for a Psychosocial Assessment to write up a bio/psycho/social/cultural/spiritual assessment of Grace. In some areas, you will not have the information to complete every detail. Where you do not have information, indicate what data/information still needs to be collected and how it relates to the overall assessment. Your assessment formulation must include a final section with explicit integration of theory informing your assessment. Although a practice-based assessment would not normally include references, you should cite to a minimum of three sources in this section of the paper to document the integration of theory for practice. You may use the theory references developed for class session 5 or other references that provide information and research for practice with this client
