As the HIM professional, you are asked to design a form as part of your new EHR implementation. Your first step is to find examples of good form design to model.

Step 1

Create a "best practices" checklist for form design, a list of 8-10 characteristics you believe a well-designed health record form must display. For help developing your list, consider all the information you've encountered during this Learning Plan; for example:

Chapter 4, pp. 354-356 of your textbook

"A Study of the Effectiveness of Information Design Principles Applied to Clinical Research Questionnaires" a Technical Communication article by Beverly Zimmerman and Jessica Schultz. Scroll to p. 182 of this document to see a detailed list of evaluation criteria for form design.

Be sure to consider integration with form automation tools and EHR as you develop your list.

Step 2

Find an example form that displays most of the principles of form design you identified in your "best practices" checklist. You may locate an example form in our textbook, find one on the internet, or even pick up a physical form from a local healthcare practitioner's office.

Step 3

In a Word document, identify the form you are analyzing:

If the form comes from our textbook, provide the figure number, title, and page number.

If the form comes from the internet, provide the form's title and a link to where you found it.

If you are analyzing a physical form, write the form title, name of the facility where you found the form and "see attached .PDF" in brackets. (And then remember to upload the .PDF of the example form with your assignment submission!)

Step 4

In the same Word document, write a few paragraphs analyzing the example form's design. What "best practices" of form design does it display? What changes would make this form design even better?

Conclude your analysis with a few sentences discussing how data is collected in the EHR.
