Case II Mr. W. is a 45-year-old white male who began drinking alcohol at age 16. He completed high school and obtained a job as a yard laborer at a railroad. He married at the age of 30, and he and his wife had four children who are now ages 14, 12, 8, and 5. Mr. W. was promoted to the position of brake operator at the time of his marriage and later became a rail yard engineer whose responsibility was to move cars within the yard, where they were then repaired. His alcohol use did not interfere with his work performance initially, although he drank heavily after work every day and was a binge drinker with his wife on days off. Five years ago, his wife stopped drinking, and 3 years ago she filed for divorce. Mr. W. 's drinking intensified, and after several accidents that occurred at work because of his drinking he was fired. He has spent 3 months in a residential treatment facility, where he was treated for Wernicke's encephalopathy; however, Korsakoff's syndrome was not present. He is now in outpatient treatment and says his goal is to return to his former job. He also expresses a desire to reunite with his former wife, although he is concerned that she has begun drinking again. Questions 1.
