How do I create a form that looks like the figure below? When I click "Yes" a textbox will show up, when I click "No" the box will disappear. How do I make the boxes thats outlined in red mandatory to fill out? and when you click submit, it'll bring you to a page where it says "The form has been successfully submitted."
