How would you write a report?

• a young woman who has been living in a community house with three other young women for some years. She has cerebral palsy and a mild intellectual disability. She managed to complete Year 10 education and would like to consider getting a job and to possibly live independently once she has the means to do so. She is able to do some things with assistance (for example dressing, feeding etc).
• report outlining how you would:
• prepare to carry out an assessment of needs and how you would implement the assessment
• identify needs (including co-existing needs) using a collaborative approach
• determine appropriate services for Julie and organise access to services
• complete reporting
• evaluate the assessment and referral process
• comply with legislation
