Some factors that lead to this can be one person taking the lead and being a hard person to disagree with, insecurity or lack of knowledge from other members in the group, and more. One way that I think could reduce groupthink would be to have everyone write their ideas down before sharing, have an anonymous way to submit thoughts and ideas, or to have a facilitator that is inclusive and open minded.

Also, having an impartial leader who encourages independent thinking can be counter productive to groupthinking.
