I need to create a VLOOKUP formula for the Price to Quote using the table array above. Please help

I was able to create the IF formual for Preferred Customer where input can only be a Y or N and also the drop down box for Product Code.

However, I am having a hard time getting the correct VLOOKUP formual in D19.
