Jubal is a 29-year-old African-American male. The following is his self-report. Jubal is the only child of a single mother who worked the early shift from 6am to 2pm six days a week at a factory. Jubal's mother began giving him Benadryl when he was a baby so he would sleep through the night. She stopped giving him the drug when he was 6. Jubal began drinking his mother's alcohol when he was 12 after she went to bed. He states that she never asked him about it but that she must have noticed that cans of beer were gone. He was drinking every day by the time he was 14 and began smoking marijuana and snorting cocaine when he was 16. He dropped out of school at 17 and went to work at the same factory as his mother. By 20 he was drinking 5-8 beers a night, and at least one in the morning to ward off "the shakes". He also snorted close to a gram of cocaine on the weekend and smoked several joints in the evening to be able to sleep after the cocaine. He had great trouble getting out of bed Monday mornings and was often late to work or called in sick. He had to go out each Saturday to get the cocaine as he could not have it in the house without using it. He reported thinking about using cocaine most weekdays and looked forward to the weekend so he could use. He says he has to do more to get the same high. His supervisor has given him two warnings in the last three months as his use of all three substances has increased and has said he will be fired if he is late again unless he seeks SUD treatment.
In the first session Jubal says he is only coming to therapy because his supervisor has said he will be fired if he does not. He complains that his supervisor "is making a big deal about being late at times". He reports that he stays later to make up the time so he works the same amount of hours as everyone else. "I would quit and go work somewhere else but I earn a good salary at my job and I don't want to take a pay cut. So, I am coming here to get him off my back."
