In a small university town, two hospitals are negotiating how to improve health information exchange between themselves and among the many physicians and other health care practitioners withinin the community. Each hospital has implemented different EHRs, most of the physicians$/$providers also have different EHR$/$EMR systems, and some coordinate with just one of the hospitals. The Chamber of Commerce, city and county government leaders, and major employers developed a task force to investigate best practices for improving population health, streamlining the continuum of care, and reducing health care costs to the patients and the medical community. The major employers also want to see an improvement in the efficiency of health care services offered in order to keep health insurance costs under control. The Chamber of Commerce has demonstratead that the area population has shifted to more retirees re$-$locating to the area because of the many amenities offered. A timeline of $18$ months has been established to complete the evaluation and to make recommendations for a public$-$private partnership to address these issues.
