Mariana is a 31-year-old, married Hispanic female who works as an advertising executive. The following is her self-report. Mariana began experimenting with alcohol at age 16. She drank at parties and began taking alcohol from her parent's mini-bar at age 17 when she began drinking on the weekend even if there was not a party. She began smoking marijuana as well at 18 when she went to college. She smoked an average of two evenings a week and would drink one or two drinks as well. She would then be very sleepy and go to bed even if she had homework to finish. She was late turning in her assignments at times but was able to keep her grades at a passing level, just not the all A's she had had in high school. At 20 Mariana's level of use increased so that she was drinking and smoking most evenings before she went to bed and all evening on the weekend. She graduated from college with a degree in business at 21 and met her husband, Alfredo, the next year. He did not smoke pot and disapproved of her doing so. Mariana stopped smoking to please her husband but her alcohol use increased to at least three to five drinks every night. She often felt "hungover" the next day and would occasionally have a few sips of beer to help her be ready for work. Alfredo became very worried about her alcohol use especially as he wanted to have children. He told her that if she did not stop drinking so much, he would leave her. Mariana then went to alcohol treatment and stopped using altogether. She and Alfredo had 2 children who are now 6 and 9. Last year Mariana was in a serious car accident which resulted in her being in the hospital for 3 months. She was prescribed opioid pain medication and sent home with a prescription for the pain in her back. She had 2 surgeries but neither was able to reduce her pain. Her doctor lowered the amount of pain medication but she states she was still in a lot of pain. Mariana began to buy pain medication from a person she met in order to keep the pain at bay. If she runs out of medication, she feels like she is getting the flu, with chills, body aches, and irritability. She has hidden her medication use from Alfredo and her children but she is more emotionally distant and preoccupied which has negatively impacted her relationship with her family.
