In the regional acute care hospital, the nursing interventions to round every hour, and the falls rate has decreased over the past two-quarters or six months. Each quarter or three months every three months data is compiled and reviewed. After six months reviewing quality improvement reports, latest reports suggest that the decrease in falls improved as a result of increasing the frequency of nursing rounds, (Aultman, 2013).

The goal is to see the number of falls and fall related injuries decrease by 70 percent by the end of the next two-quarters, or after six months. Nursing rounds or checking on patients every two hours are inadequate at best. The reason the current rounds of every two hours are insufficient because patients are at risk for falling. The proposed solution for the prevention of falls or potential fall and injuries in the acute care setting is to review current nursing rounds. The staffing in the regional acute care hospital is six nurses to one patient in the acute care setting. The ratio does not allow time to assess the patients more than one time every two hours. The potential solution in addition to a tool currently in use to assess falls risk is to increase nursing rounds to every hour instead of every two hours Blegan (2006).
