In therapeutic work the accepted approach when helping assess the clients situation at the various stage of the encounter is for the practitioner to observe "signs and symptoms" and record observations received:Signsare what the practitioner see's hears and interprets -Symptoms on the other hand are what the client expresses- says and reports:*How would you check to make sure that what you have observed and what the client has offered and reported is objectively correct and has not been contaminated by noise from your own sensory and perceptual expectations?
