In this project, you will be required to assess the diverse and multi-faceted needs of people within the community services sector and determine both internal and external services required to meet those needs.

• Assess and appropriately respond to the requirements of at least 3 people presenting with co-existing needs.
• Use analytical and critical thinking skills in each case by:
  • Applying a strengths-based approach to assessment.
  • Analysing information about co-existing issues.
  • Making evidence-based judgements about the person's needs.

This assessment task may be completed in a classroom, workplace, or independent learning environment. Submissions must be made in written form unless the RTO Manager approves reasonable adjustments. Submissions must be completed via the online learning management system (LMS) unless approved by your assessor.

Task 3 is divided into (5) parts:

• PART A - Case assessment
• PART B - Reflection with assessor
• PART C - Assessment changes
• PART D -Referral
• PART E - Seek feedback and improve performance

Part A Case assessment Vou will conduct a thorough case assessment of the requirements of three [3) people with coexisting needs. To do this vou will need to: 1. Gather existing information about each person ov reviewing: - The current client file [see 'client file\" document} - The learner resource with details of programs and services 2. Use a variety of sources to identifv and research the issues that this person is facing. Specifical Iv: I ldentifv a minimum of two {2) coexisting needs the person is experiencing . How these needs impacts on the person - The issues the person is facing 3. Based on your assessment and analvsis of the person's co-existing needs complete the 'file review\" template. You are required to complete all parts of the template and submit for assessment. PART B - Reflection with assessor Complete a consultation via email with your assessor on the analysis your completed of each client's file. 1. Send your completed template from Part A for all three (3) clients via email to your assessor. 2. Your assessor will review the completed template and provide written feedback. 3. The reason for referral . Presenting Issues . Assessment information (eg mental health, drug and alcohol, vocational/educational, physical health, including past/current risk assessments) 2. Upload the three (3) completed referrals below. IMPORTANT: You must follow the organisation's referral process.
